You can also print one off for yourself so the class can get to know more about you as their teacher. This All About Me questionnaire is a great way to break the ice with new students at the start of the school year and a fun way for them to share about themselves. If your students are confident enough, once their sheets are filled out they can stand up and present their answers to the class to introduce themselves and what they like, or you can display them on a wall so they can all get to know each other better.
The questionnaire can be used with children starting in kindergarten, foundation or prep {they can verbalise their answers to their teacher or teacher aide who can fill the sheet in for them if they can't write yet} all the way through to students in their last year of primary or elementary school.

Each question on the sheet has a corresponding image or section the children can colour in so they can get creative decorating their sheet. The a picture of me polaroid section can be used for them to draw a picture of themselves, or you can cut out a printed photo of them and tape or glue it into the picture section.
The questionnaire is also a great way for younger students to practice their letter formation and handwriting skills, as well as developing their language, and building their fine motor skills as they write and decorate on the page.
All About Me Printable Questionnaire
To fill in the questionnaire you will need:
- All about me template
- White paper or cardstock
- Pens, pencils or crayons
- Laminator {optional}
1. Print out the All About Me questionnaire template onto paper or cardstock. The printable comes in both US and UK/AU spelling and you can print as many copies as you need. If you print on paper and want to keep the questionnaire as a keepsake, I recommend laminating the sheet once the children have finished filling it in.
2. Get children to answer the list of questions. If they can't read or write themselves yet ask them the questions and record the answers for them. Older children will be able to fill in the questionnaire themselves.
3. Fill in the photo section. Either have the children draw a picture of themselves directly onto the sheet, or use a copy of a photograph of them and glue it onto the sheet.
4. Laminate the finished questionnaire {optional}. I recommend this step if it was printed on paper, to allow for longevity so it doesn't get ruined over time, or you can frame it.
5. Display completed All About Me pages in class, or collate them into a class folder. Create an All About Me wall in the classroom with all students completed questionnaires on display, or collate all the sheets into an all about our class folder. This is a fun way for students to get to know each other better and the sheets can be sent home as a keepsake for parents at the end of the year.
